3. The URL(s) of the infringing material on Pulse.
4. Your contact information (address, phone, email).
5. A statement that you have a good-faith belief the use is not authorized by the copyright owner, its agent, or the law.
6. A statement, under penalty of perjury, that the information is accurate and you are authorized to act on behalf of the copyright owner.

If you believe content was removed in error, you may submit a counter-notice to the same address including the required statements under 17 U.S.C. § 512(g)(3).

We may terminate, in appropriate circumstances, the access of users who are repeat infringers.
